Eligibility criteria for subjects with clinical coronary disease in the ADVANCE study
Cohort 1 (n = 500): “Young Cases with clinically significant CAD”
Inclusion
- Evidence in the electronic databases of any of the following in the time period 1/1/1999-12/31/2003: a rise in cardiac enzymes with a discharge diagnosis of CAD, a coronary revascularization procedure, or a diagnosis of angina with a coronary angiogram documenting at least one stenosis > 50%.
- Age on date of first hospitalization for qualifying event: 18 to 45 for males, 18 to 55 for females
Exclusion
- Evidence in the electronic databases (going back to 01/01/1996) of any of the following: CAD related diagnosis at anytime prior to the date of hospitalization, cancer, cirrhosis, renal failure, a bone marrow or organ transplant, entry in “no contact list”, no assigned PCP, and remote distance from the study clinics.
- A report during a screening interview of an inability to speak and write English, or any of the above exclusion criteria except a history of CAD prior to membership in KPNC.
Cohort 2 (n = 468): “Older cases with incident stable exertional angina”
Inclusion
- Evidence in the electronic databases in the time period 10/28/2001-12/31/2003 of an outpatient diagnosis of stable angina (ICD 9: 413.xx)
- Age on date of first diagnosis of stable angina in the databases of >18 years
- Evidence in the electronic databases (going back to 01/01/1996) of any of the following: CAD related diagnosis or nitroglycerin prescription at anytime prior to the diagnosis of stable angina minus 6 months, dialysis, a bone marrow or organ transplant, entry in “no contact list”, no assigned PCP, and remote distance from the H3 study clinics.
- A report during a screening interview of any of the above exclusion criteria, symptoms of CAD at anytime prior to the date of diagnosis of stable angina minus 6 months, or an inability to speak and write English.
- Eligible & willing to participate in the study as a young case (see eligibility Criteria of Cohort 1
Cohort 3 (n = 924): “Older cases with incident acute myocardial infarctions”
Inclusion
- Evidence in the KPNC electronic databases in the time period 10/28/2001-11/04/2003 of a rise in serum cardiac enzymes (Troponin I level >= 4.0ng/m, OR CK-MB >=5.6 ng/ml and CK-MB% >=3.3) during or within 72 hours of hospitalization. Subjects with a rise in CKMB only had to also have a discharge diagnosis of CAD (ICD 9: 410.xx).
- Age on date of cardiac enzyme elevation of > 45 years for males and > 55 years for females
Exclusion
- Evidence in the electronic databases (going back to 01/01/1996) of any of the following: CAD related diagnosis or nitroglycerin prescription at anytime prior to the date of hospitalization minus 14 days, dialysis, a bone marrow or organ transplant, entry in “no contact list”, no assigned PCP, and remote distance from the study clinics.
- A report during a screening interview of any of the above exclusion criteria, an inability to speak and write English, or symptoms of CAD at anytime prior to the date of hospitalization minus 14 days.
