Mission Neighborhood Health Center Adolescent Clinic
Part of the community clinic system in San Francisco. The referral hospital is San Francisco General Hospital. The Adolescent Clinic meets weekly on Monday afternoons and serves a diverse patient population. Comprehensive primary care services are offered, as well as help for special adolescent needs such as eating disorders, family planning, and smoking cessation counseling. Dr. Seth Ammerman attends the clinic along with a female provider, a social worker, and health educators. A dietitian is available by referral. HIV testing and counseling are also available. To make an appointment please call 415.552.1013 Ext 339 and ask for Monique Hosein, teen clinic coordinator. All insurances are accepted. Patients without insurance are charged on a sliding scale and all uninsured patients are considered for eligibility for MediCal, Healthy Kids, or Healthy Families.

Packard Children’s Hospital Adolescent Health Van
Packard Children’s Adolescent Health Van is a health clinic on wheels that provides comprehensive primary care services to medically underserved teens. The Adolescent Van offers services for youth, ages 12-21. Services include physicals, immunizations, pregnancy testing, pelvic exams, sick care, HIV counseling and testing, family planning, nutrition counseling, mental health counseling, and health education. Dr. Seth Ammerman from the Division of Adolescent Medicine is the Medical Director of the Adolescent Van. Van personnel include Dr. Seth Ammerman, a female nurse practitioner, a social worker, and a registered dietician.
